Records Request Information

Copies of police reports, including accident reports, are available in the Records Division, Monday through Friday, 9:00 a.m. to 5:00 p.m., excluding legal holidays. Photo ID and payment of the fee are required.

  • You will need to complete the records request form at the police department, unless the completed form is notarized.
  • You can bring a notarized copy of the request form to the police department or email a notarized form to: records@tremontoncity.gov

Reports

$ 10.00 minimum for up to 15 pages
$   .25 for each additional page

Photos/CD

$ 20.00 minimum for up to 5 photos (4×5)

Audio/Video

$ 20.00 minimum for tape and 1 hour of dubbing
$ 10.00 for each additional hour of dubbing

Most of the records maintained by the Police Department are classified as private, protected or controlled, in accordance with the Government Records Access and Management Act and as such are only released to individuals meeting certain criteria.  At the time a request is received, careful consideration will be given to ensure all federal, state and local laws are abided by.

Letter of Good Conduct

The Police Department will provide a letter of good conduct to citizens of Tremonton City free of charge, provided there is no record at the Tremonton Police Department of a criminal conviction.  These letters are generally used for individuals wishing to obtain a Visa for international travel.
